You can think of concerns, issues, or problems as a special type of process, and just like processes they need to be untangled and focused.
When they are not untangled, they tend to make the organization go in circles as changes help part of the concern but are counterproductive for other parts.
When you understand a concern and you can state it simply and show the future state where the concern is addressed you are ready to deal with the concern.
Don’t be lazy and take concerns at face value. At face value, you will not get to the root cause of the issue.
As an example, if you are dealing with concerns like an application interface is no good you need to determine what the concern is. Is it the look and feel? The responsiveness? The organization of information?
Determine the real concerns and what the future state would look like.
